I/We authorize the management to perform a credit investigation to verify the above reported information, which pertains to my/our credit and financial responsibility and agree to pay the application fee of $25.00.

Note: Applications will not be processed without the $25.00 application fee. It MUST be paid to the manager or included, if mailed to the QSP Inc. office; and, copies of income verification MUST be attached. Checks and money orders should be made payable to QSP Inc. A resale inspection of the home MUST be scheduled and completed by the park manager as part of the application process. This application needs to be COMPLETELY filled out, in order to be processed. Residency prior to application and approval will result in residents eviction and home's removal from the community.
